Financial and Insurance
We provide each patient with a treatment plan, which includes a breakdown of all the applicable fees. It is our goal to inform our patients of all costs prior to any procedure. Our office requires payment in full at the time of service. For those patients with dental insurance, co-payments and deductibles are also required at the time of service.
We gladly accept: Cash, Checks, American Express, Mastercard, Visa, and Discover.
We will review your medical insurance and explore all the available options to help you achieve your dental health goals. Our goal is to work with our patients and come alongside them to achieve their health goals with confidence. Many patients have had life opportunities open up thanks to their beautiful new smile.
If you have dental insurance, we’re happy to complete the necessary forms for your claim as a courtesy to you. However, your insurance is a contract between you and your insurance company and you are responsible for your entire bill regardless what your insurance pays. We are only a third party providing a dental service to you. Amherst Village Dental requires that you be responsible for your co-payment and deductible at the time of your procedure. After insurance forms have been filed and benefits have not been received within 60 days from your insurance company, the entire balance of treatment becomes your responsibility as a patient. A refund will be given when the benefits have been reimbursed from the insurance company. This office cannot render services on the assumption your charges will be paid by your insurance company.
Following our original submission, any disputes of coverage must be made by the patient to your insurance provider. All remaining balances that remain after submission of a claim are the financial responsibility of the patient.
As a service to our guests, we are pleased to offer Wells Fargo, GreenSky®, Lending Club Patient Solutions, and CareCredit healthcare card. Cancellations by email or text are not sufficient. Canceled appointments without a reasonable notification to our office may be charged a fee. We reserve the right to charge a reservation fee for rescheduling.
